You cannot have gap cover without medical aid. Gap cover is an FSCA-regulated insurance product that operates only as a top-up to a CMS-registered medical scheme. If you cancel or lose your medical aid, your gap cover lapses automatically. The 2017 Demarcation Regulations enforce this restriction.

What gap cover actually pays for

Specialist co-payments. Medical aids pay specialists at "scheme tariff" β€” typically 100-300% of the SA Medical Association scale. Specialists often charge above that, leaving a co-payment shortfall. Gap cover plugs this difference for in-hospital admissions.

Anaesthetist shortfalls. A particularly common gap β€” anaesthetists often charge well above scheme tariff and the shortfall on a single surgery can run R5,000-R30,000+.

Hospital sub-limits. Some medical aid plans cap certain in-hospital procedures at a Rand amount; gap cover can fund the excess (subject to its own sub-limits).

Cancer / severe illness extension. Most gap policies include or offer cancer cover β€” additional lump-sum payouts for cancer diagnosis. Cover amounts vary R50,000-R200,000.

Casualty cover. Some gap policies cover casualty visits that medical aid doesn't pay (typically R3,000-R5,000 per incident).
