Who can join GEMS?+
GEMS (Government Employees Medical Scheme) is a restricted scheme open only to employees of the public sector — national government, provincial government, public entities and parastatals. If you work for the private sector or are self-employed, you cannot join GEMS — see Discovery, Bonitas or Momentum among the open schemes.
How much does GEMS cost in 2026?+
Tanzanite One (cheapest) ranges from R1,698 to R2,881/month for the main member depending on salary band. Top-tier Onyx ranges R7,673 to R8,621. The government employer typically pays around 70% of contributions; for the lowest salary bands (levels 1-5) the employer may subsidise up to 100% of the Tanzanite One contribution.
What is the GEMS employer subsidy in 2026?+
The Minister for Public Service and Administration approved a 4.5% subsidy increase from 1 Jan – 31 Mar 2026 (delayed to Feb backdated to Jan), plus 0.5% from 1 Apr 2026 – 31 Mar 2027. The base structure is unchanged: the government as employer typically pays around 70% of an employee's contribution; salary levels 1-5 can receive up to 100% employer subsidy on the Tanzanite One plan.
Is the GEMS 9.8% increase reasonable for 2026?+
9.8% is the highest increase among the major schemes for 2026. COSATU and the Public Service Co-ordinating Bargaining Council unions publicly rejected it. The CMS asked schemes to cap 2026 increases at 3.3% plus reasonable utilisation; GEMS justified the higher figure on the basis of utilisation patterns in its specific membership. CMS also rejected GEMS's request to relax the 25% reserve requirement, meaning the scheme had to fund increases entirely from members.
